Dobson and Associates has developed a unique and comprehensive Disease Management Program which can be customized to meet the needs of the client. Our program involves telephonic and face-to-face contact by appropriate health care professionals. We utilize the services of registered nurses, pharmacists, licensed clinical social workers, respiratory therapists, health educators and dieticians to enhance and maximize training and educational opportunities for members with chronic diseases. Primary Disease Management Services are provided by a registered nurse disease management specialist who will then coordinate with the other health care professionals as needed to meet the individual needs of the member. The goal of Disease Management is to promote a healthier life style and to improve the wellbeing and medical management for the member with a chronic illness. This in turn will reduce lost work days for illness, reduce overall medical expenses over time, and increase productivity.

A Disease Management Specialist will coordinate all services for the client and members. The Disease Management Specialist will:

∙           Direct care on site Disease Management/Case Management for client’s who are enrolled in the program;
∙           Provide face-to-face assessments with the member and their family;
∙           Contact and follow-up on clients with chronic disease states to promote changes in lifestyle and regular attendance to medical appointments;
∙           Organize educational programs for employees and management staff;
